Consider a situation, when we need a survey on a topic. So i need to create a dialogue box in between where there must be three options to a question i posted in survey. Three options must be 1) yes 2) No 3) if others , specify below where the user will give his inputs.

Please help me out. its very urgent. Thanks in advance
